ABSTRACT

The prognosis of malignant tumors of the digestive system is still a matter of close attention in the medical and health field. At present, evaluating the prognosis of patients with digestive system malignant tumors is mainly based on the TNM staging of the tumor after surgery. As one of the ten characteristics of cancer, inflammatory indicators have certain guiding significance for the prognosis and treatment of patients with malignant tumors of the digestive system. As a new type of inflammation index, the systemic inflammatory response index is significantly better than other traditional inflammatory indexes in evaluating the prognosis of patients with digestive system malignant tumors. This article reviews the related studies on the systemic inflammatory response index in evaluating the prognosis of digestive system malignant tumors.

ABSTRACT

Objective To investigate the prognostic value between gastric cancer and esophageal adenocarcinoma staging system from International Union Against Cancer (UICC) / American Joint Committee on Cancer (AJCC) 8th edition in treatment of the patients with advanced Siewert type Ⅱ adenocarcinomas of esophagogastric junction (AEG) in China. Methods A total of 67 patients who were pathologically diagnosed as pT3-pT4 Siewert type Ⅱ AEG in Shanxi Provincial Cancer Hospital from November 2012 to September 2013 were enrolled. The stage distribution and survival of the patients in two staging systems were compared. The homogeneity and discriminative power of the two staging systems were compared by using likelihood ratio chi-square test and linear trend chi-square test. Log likelihood and C-index were used to evaluate the prognostic value of the two staging systems. Results The stage distribution in gastric cancer staging system of 67 patients was ⅡA (16.4%), ⅡB (14.9%), ⅢA (31.3%), ⅢB (34.3%), ⅢC (3.0%), and 5-year survival rates ofⅡA, ⅡB, ⅢA, ⅢB were 81.8%, 70.0%, 39.1% and 36.6%, respectively (χ2= 8.221, P= 0.042); the stage distribution of esophageal adenocarcinoma staging system was ⅡB (22.4%), ⅢB (40.3%) and ⅣA (37.3%), and 5-year survival rates were 80.0%, 43.7%and 40.0%, respectively (χ2=7.671, P=0.022). The likelihood ratio chi-square value and linear trend chi-square value of the gastric cancer classification system were more than those of esophageal adenocarcinoma staging system (7.855 vs. 6.692, 6.051 vs. 5.351), and Log likelihood estimation value of the gastric cancer classification system was less than that of esophageal adenocarcinoma classification system (241.324 vs. 242.045), and C index of the gastric cancer classification system was more than that of esophageal adenocarcinoma staging system (0.7289 vs. 0.7215). Conclusion UICC/AJCC 8th edition gastric cancer TNM staging system is superior to esophageal adenocarcinoma TNM staging system in predicting the prognosis of advanced Siewert typeⅡAEG.

ABSTRACT

<p><b>BACKGROUND</b>Herpes simplex virus thymidine kinase phosphorylates ganciclovir to ganciclovir monophosphate, which is then converted to ganciclovir triphosphate by endogenous cellular nucleoside kinases. The ganciclovir triphosphate acts as a DNA chain terminator due to the lack of a functional 3'-OH group and terminates the process of DNA replication, hence leading to cell apoptosis. At present, HSVtk gene usually acts as suicide gene to kill tumor cells. The aim of this study was to investigate the selective cytotoxicity of the herpes simplex virus thymidine kinase/ganciclovir (HSVtK/GCV) suicide gene system controlled by the a-fetoprotein (AFP) promoter on hepatocellular carcinoma (HCC) cells in vitro.</p><p><b>METHODS</b>pAFP-HSVtk-IRES2-EGFP recombinant plasmid vectors driven by the AFP promoter were constructed. HL-7702 liver cells, HUH-7 HCC, and HepG2 HCC were transfected with the recombinant plasmids. HSVtK gene expression was detected using Western blotting analysis. HepG2 cells line stably expressing HSVtk gene was selected by G418 reagent. The cytotoxicity of HSVtK/GCV suicide gene system on hepatoma cells was measured by CCK-8 reagents when different doses of ganciclovir were added.</p><p><b>RESULTS</b>Plasmid pAFP-TK-IRES2-EGFP-expressed HSVtk gene was constructed successfully. HSVtk gene expression level was significantly higher in AFP-positive hepatoma cells than in AFP-negative liver cells. After G418 selection, a HepG2 cells line stably expressing HSVtk gene was acquired. With the increase of the dose of ganciclovir the optical density at 450 nm of HepG2 cells stably expressing HSVtk gene gradually decreased (P < 0.05).</p><p><b>CONCLUSION</b>The HSVtK gene-specific expression in hepatoma cells as well as the cytotoxicity of the suicide gene system in HepG2 cells provided the basis for the targeted gene therapy of HCC.</p>

ABSTRACT

ObjectiveTo compare the effect of salt bag compression and arterial compressor haemostat for patients after cerebral angiography.Methods Four hundred and ninety-five patients undergoing cerebral angiography were divided into two groups according to the methods of arterial compression:salt bag compression group (243 cases) and arterial compressor haemostat group (252 cases) in which 131 patients used the YM-GU-1229 arterial compressor haemostat and 121 patients used the NZ-1 arterial compressor haemostat.The associated complications within 3 days were recorded.ResultsThe incidence of complications in salt bag compression group was 3.70%(9/243),in arterial compressor haemostat group was 9.92% (25/252),and there was significant difference between the two groups(P < 0.01 ).The incidence of complications in YM-GU-1229 arterial compressor haemostat was 12.21% ( 16/131 ),in NZ-1 arterial compressor haemostat was 7.44%(9/121 ),and there was no significant difference between the two arterial compressor haemostats (P > 0.05).ConclusionThe incidence of complications of arterial compressor haemostat is higher than that of salt bag compression.
